After Admission Individuals are assigned to a treatment team composed of a psychiatrist team leader, a psychologist, social workers, nursing personnel, an activity therapist, and other staff as relevant. Discharge planning with Individual, family and the Community Services Board begins with admission and culminates with referral back to the community for follow-up treatment based on the individual needs of each Individual. A Therapeutic Pass may be given to an Individual when it appears that a period of time spent in the community would clarify the Individual’s readiness for discharge. A single pass of a few hours up to two days may occur as needed to consolidate placement planning.
