All visitors are required to go to Reception before visiting in the homes or any other areas on campus. All visitors will be screened and will be provided a visitor’s badge.

We want to remind you about some ways to help to protect you and your family from getting sick:
- Stay at home if you have symptoms
- If children are sick please do not send them to school
- Frequent hand-washing with soap and water or using alcohol-based hand cleaner
- Use tissues to cover mouth and nose when coughing or sneezing and dispose of the tissue in a wastebasket. If you do not have a tissue and need to cough or sneeze use your upper sleeve, not your hands.
- Stay at least 3-6 feet from someone who appears to be ill.
- Avoid touching eyes, nose, and mouth
- Get plenty of rest
- The symptoms of COVID-19 can range from mild to severe and may appear 2-14 days after exposure.
- Fever
- Cough
- Shortness of breath
- Older adults and people with underlying chronic medical conditions seem to be at higher risk for more serious disease course with COVID-19
- Call your Doctor if you develop symptoms and have been in close contact with a person known to have COVID-19 OR have recently travelled from an area with widespread or community spread of COVID-19

IMPORTANT INFORMATION REGARDING THE CONSENT FORM PROCESS FOR ON-SITE COVID-19 VACCINATION CLINICS.
OVERVIEW:
The process and requirements for COVID-19 vaccination consent have been finalized and are now available to share externally with facilities. The attached guide outlines the various versions of consent forms as well as the requirements for each method of obtaining consent to receive the COVID-19 vaccine. This is for the CVS Pfizer vaccine and for the DBHDS provided Moderna vaccine.

Versions of Consent Forms:
- COVID-19 CVS Vaccine Consent Form: *PREFERRED* physical, triplicate form shipped directly to facilities (alone or with RP cover sheet)
- Electronic, editable PDF (two options):
- COVID-19 Vaccine CVS Intake Consent Form
- COVID-19 Vaccine Consent Responsible Party Form (abridged)
–Requirements for CVS Clinics:
- Three (3) copies of signed, completed consent forms (whether it’s gathered electronically or paper form).
- If a responsible party completes the electronic cover sheet, we need three (3) copies printed, to attach one to each copy of the physical COVID-19 Vaccine Consent Form.
- Two (2) complete sets of consent forms will be required for each individual participating in the vaccination clinic – one set for the first dose/clinic and a second set for the second dose/clinic.
