Dental Program Mission
The Dental Program’s mission is to provide dental evaluation, education, exams, and treatment to individuals with intellectual and developmental disabilities who do not have access to basic dental care.

To request Dental Services, please fill out the online form below and one of our dental team members will be in contact with you as soon as possible. Please ensure you only submit one referral per individual.
