Licensing Complaints

DBHDS values the citizens of Virginia and appreciates the input provided by community members and service providers. DBHDS is committed to delivering the highest quality of service to all stakeholders. Concerns and complaints will be handled promptly and in accordance with established procedures.

Submit complaints related to a licensed provider through the CONNECT portal.

Please note that DBHDS will only investigate complaints alleging violations of the Rules and Regulations For Licensing Providers by the Department of Behavioral Health and Developmental Services [12 VAC 35 ‑ 105] or Regulations for Children’s Residential Facilities 12VAC35-4  While DBHDS may not investigate certain matters directly, allegations may be referred to the appropriate enforcement agency.

Helpful Resources for Complaints or Concerns Outside of DBHDS

Department of Social Services (DSS)

Adult Protective Services (APS) – reports of abuse, neglect, and exploitation of adults 60 years of age or older and incapacitated adults aged 18 or older.

Child Protective Services (CPS)- reports of abuse and neglect; conducting investigations to determine the validity of the CPS reports; and providing services that enhance child safety and prevent further abuse and neglect to families and children.

Assisted Living Facilities (ALF)- reports related to a non-medical residential setting that provides or coordinates personal and health care services, 24-hour supervision, and assistance for the care of four or more adults who are aged, infirm or disabled.
