Resources for Community Housing Guides 

Community Housing Guide (CHG) is a service available in the Medicaid Waivers for people with developmental disabilities. A Community Housing Guide supports an individual’s move to independent housing by assisting with transition and tenancy sustaining activities. The following documents explain what the Community Housing Guide service is, how the service works, and how CHG providers collaborate with others to help people live independently. 

Providers that offer Community Housing Guide services must have staff complete two required trainings before billing for services: 


DBHDS Independent Housing Curriculum Modules 1-3 for Community Housing Guides 

DBHDS has developed tools Community Housing Guides can use to assist individuals with transitioning to independent housing and sustaining tenancy.
