PC ISP v4.0 Resources (2024)
The five part Individual Support Plan (ISP) for DD waivers, located in WaMS, is updated and improved periodically. The current version is 4.0. Most of the materials in this section reflect that version, although a few general resources are based on an earlier version. All materials in this section, whether videos, printed resources, or examples of how to best write elements of the ISP, are designed to aid Support Coordinators and DD services providers.

Therapeutic Behavioral Consultation – WaMS ISP element recording
(July 2024) – This training enables trainees to become familiar with expectations regarding connecting an individual with a behaviorist in 30 days, as well as how to utilize the behaviorist search engine resource.

Electronic Home-Based Services (EHBS) Resource
Electronic Home-based Services (EHBS) Optional Assessment Form (10.31.25) - optional assessment tool to aid planning teams in determining an individual’s ability and desire to participate in and benefit from EHBS. It can be completed by any member of the team except the EHBS provider
