A support coordinator shall complete a minimum of eight hours of training annually in one or more combinations of areas described in the knowledge, skills, and abilities section of the regulations above and shall provide documentation to his supervisor that demonstrates that training is completed. The documentation shall be maintained by the supervisor of the support coordinator in the employee’s personnel file for the purposes of utilization review. This documentation must be provided to DMAS and DBHDS upon request.

SC Modules 1 – 10   

Behavioral health, substance use disorder, intellectual disability, and developmental disability support coordinators/case managers hired on or after April 1, 2019, are required to complete all modules and must demonstrate their knowledge and understanding of the content by passing a competency-based post-test for each module within 30 days of employment.  

On-site Visit Tool 

The On-site Visit Tool (OSVT) must be completed at one face-to-face visit monthly for people receiving ECM and once quarterly for people receiving targeted case management (TCM).  It must be uploaded into the Waiver Management System (WaMS) under the “Person’s Information” attachments section. This form provides a means to ensure consistency is applied when assessing for any change in status and to confirm that the services are implemented appropriately.  

These resources include the form along with a training video that covers key concepts including defining “change in status” and “services being implemented appropriately.”

Crisis Risk Assessment  

The Crisis Risk Assessment is used to gather information that might show a person is at risk of a crisis or hospitalization. It also helps Support Coordinators/Case Managers make timely referrals to the REACH program when needed.  

Support Coordinators/Case Managers must use this tool at the initial intake and during face-to-face visits for anyone with a DD diagnosis. If the person is already receiving active services from REACH, the form does not need to be completed, but the Support Coordinator/Case Manager should still ensure the person’s supports and services match their current needs.

Housing  

Affordable housing is essential to DBHDS’ mission to build a life of opportunities for all Virginians. DBHDS supports individuals with developmental disabilities to explore independent housing options in their communities and to identify the supports they need to move into their own homes. DBHDS administers housing resources specifically for individuals receiving services through the DD waivers and those on the DD waivers waitlist.  

These resources include funds for long-term rental assistance as well as Flexible Funds for one-time housing costs such as security and utility deposits. The DBHDS Regional Housing Coordinators are available to assist individuals, their families and support teams with navigating a broad array of housing resources and opportunities.

 These materials are intended to support staff in building the knowledge and skills needed to deliver high quality services.   

DD SC Handbook

Support Coordination/Case Management is the core service that Virginians with developmental disabilities and behavioral health disorders use to help navigate and make the best use of Virginia’s publicly funded system of services. It is the one service that is of critical importance to all dimensions of the services system. The purpose of this handbook is to guide SCs/CMs in all aspects of their work.

Enhanced Case Management

The CSB DD Support Coordinator/Case Manager must provide more frequent face-to-face visits (i.e., monthly) to individuals who are on the DD waiver or the DD waiver wait list receiving Commonwealth Coordinated Care (CCC) Plus waiver who meet certain criteria.
