In 2016, Virginia implemented a redesign of its Developmental Disability Waivers. This PowerPoint describes the changes that occurred during that transition, the driving factors behind the changes, and the services that were available when the redesign first occurred.

Person Centered ISP Training Modules

The PC ISP training is now in the Commonwealth of Virginia Learning Center (COVLC). Once in the system, search for ISP.

Recommended for all support partners who assist with developing the Person Centered ISP.
The Person Centered Individual Support Plan Training Modules and accompanying samples were developed utilizing the ISP version 2.0. The training is comprised of five modules: Person Centered Practices (1), Before the Meeting (2), Identifying Risk (3), The Planning Meeting (4), and After the Meeting (5). The majority of the content is found in the notes section of the PowerPoint. Please download the PowerPoint to view the content. The modules are currently being updated to reflect Version 3.0.

This training is in preparation for the Provider Readiness and Education Program (PREP). This training will cover information regarding the Developmental Disability (DD) Waivers, how to access a DD waiver, and the services and supports available under the DD waivers.

Outcome Development (Part III) Training

This in-person training will provide an in depth look at how to recognize identified and unidentified risks, how to locate those risks in the ISP and how to develop person centered outcomes as part of the Part III of the ISP that address person-centeredness, risk and measurability. This training utilizes practice exercise and group discussions to help facilitate a through and concrete understanding of the content.

This in-person training will utilize a combination of lecture and group work to understand what is required of the various elements of the Part V- Plan for Supports. By the completion of this training, each participant should have a thorough understanding of how to take their agency’s assigned outcomes (outcome statement, key steps and services to get there, target date) from the Part III and develop the Part V.

Training in person centered thinking consists of 2 days of exercises where the participants acquire core skills in person centered thinking such as: The importance of being listened to and the effects of having limited positive control; Learning to “listen” to people who don’t communicate in traditional ways; The role of daily rituals and routines; Discovering what is important to people; Sorting what is important for people from what is important to them; Respectfully addressing significant issues of health or safety while supporting individual choice and control; Developing goals that help people get more of what is important to them while addressing issues of health and safety.

This interactive training will provide information and skill development activities. Helping people develop their interests, the audience will experience several techniques and tools for determining where a person might start to be connected, and how to plan for the support needed for success.

**Prerequisite: Person Centered Thinking training (2 day)**

Learn skills necessary to facilitate person centered information gathering and use the information gathered to develop outcomes. The training is co-presented by a person with a disability and is designed to be interactive with the audience to build skills that strengthen person-centered practices.
